❄️FROZEN SHOULDER (adhesive capsulitis) is a condition that occurs more commonly in males and females over the age of 40, although all ages can be affected.
Basically the shoulder capsule thickens and becomes tight, causing stiff bands of tissue (adhesions) to develop. In many cases there is less synovial fluid in the joint.
The hallmark sign of this condition is not being able to move your shoulder, either on your own or with someone else's help.
Frozen shoulder develops in 3 stages:
1️⃣ Freezing: In the "freezing" stage you slowly develop more and more pain. As the pain worsens, your shoulder starts to lose range of motion. This stage typically lasts anywhere between 6 weeks and 9 months 😔
2️⃣ Frozen: Stiffness remains in this stage, but painful symptoms may actually improve. This stage usually lasts between 4 to 6 months.
3️⃣ Thawing: Shoulder motion starts to improve during this stage. Complete return to normal or close to normal strength and motion typically takes between 6 months and 2 years.

The more time I spend increasing my experience as a Physical Therapist and reading research, the more I believe that PTs should focus less on drills that look like they belong in the circus, and focus more on implementing an intelligently designed strength training program 🏋️♂️
Too often, PT involves patients doing things like single leg balance drills on wobble boards with biofeedback, complex self-mobilization drills that you almost need to be a contortionist to perform, and multi-task exercises with specific balloon breathing techniques (this might be a touch of exaggeration, but sadly isn't far off from what I see) ❌
Do these types of drills SOMETIMES have a place in rehab? Yes. But these things should be the vast minority of what we do 💁🏻♂️
Focus on getting your patients stronger while instilling in them the ideals of the resiliency of the human body, an anti-fragility mindset, and self-empowerement 🤘🏻
If you do this, you'll get them a lot better, a lot faster (and your patients will probably fall off equipment a lot less, which is good if you're a fan of not getting sued)

Let me tell you about a little bugger of a muscle called Subscapularis (aka: sub-scap)⠀
As a part of the rotator cuff, this muscle is a leading contributor to shoulder pain, neck pain, and even arm/wrist pain. Sometimes, the muscle gets tight and pinches on your Ulnar Nerve, causing numbness/tingling in your fingers. Many people mistake a tight subscap as Carpal Tunnel Syndrome. 😮😮⠀
I'm not a doctor, so I can't diagnose anything, but I do know muscles. If this muscle feels tight, I can help to relieve the tension in it. Most people feel a decrease in the symptoms I mentioned above.⠀
Self care for sub-scap: Take a foam roller or even an tennis ball, lay on your side and place the ball under your armpit, right along the ribs. This can be very intense, so go slow and take it easy.⠀

I like this movement for a few reasons. First, it is simple to perform and has a fun flow to it. I imagine I am drawing back a bow and pausing in perfect stillness each time.
Second, it trains loaded shoulder extension, which is something I am currently working to improve upon. It also works big toe extension, another area I could use some love.
Third, it can be progressed for more intensity and you can add a float in the middle to practice your handstands. I often use it for my high intensity interval training at the end of a workout with a 15 sec on 15 sec off work to rest ratio (or a 30 on 30 off).
Fourth, it is a great activation movement for the transversus abdominis (TVA), which is a very important group of tissues that helps support and stabilize the spine. Muy importante if you happen to sit down often ahem everyone. 🦁
It is also easily integrated into other movement flows as you expand on your practice BUT start with snacky snacks. Just bust out one minute of this every hour on the hour, and after a day of work, you will have collected 10 minutes of movement while also breaking up the stagnation of long term sitting. This is what my patient said out loud when she stepped up to deadlift her kettlebell today. Gila is her granddaughter. She knows that she wants to be able to lift her without stenotic back pain.
I love her approach. Unless you're in a weightlifting competition, you should be training FOR A PURPOSE OUTSIDE OF SIMPLY MOVING THAT WEIGHT.
So, the next time you're dragging yourself to the gym or pumping yourself up for a "dreaded" workout, change your mindset. Think of the life events and activities that you love to do, and train with the intent to make those things easier and more enjoyable.
Train for a lifetime, but train for LIFE.
