Rathleff et al (2014), was the first landmark publication to show this variation of a heel raise to facilitate a decrease in pain and increase in function. More importantly to start treating Plantar Fasciopathy similar to a tendinopathy.
What you might not know about how these raises were performed?
1️⃣ Every heel-rise consisted of a three second concentric and eccentric phase with a 2 second isometric pause at the top.
2️⃣The high-load strength training was slowly progressed throughout the trial.
3️⃣12 repetition maximum (RM) were prescribed for three sets. After two weeks, they increased the load by using a backpack with books. As well as changing the prescription to 4 sets of 10 repetitions (of a 10RM).
4️⃣After four weeks, the prescription increased to 5 sets of 8 reps (of a 8RM). Subjects were instructed to continue adding books/weight to the backpack as they got stronger.
Understanding your patient and loads they subject to their feet should still be the foundations of how you treat PF pain. What this article adds beyond the great exercise is the duration of treatment (as a general guide) and this can be used to educate your patients.
