Друзья, логическим завершением серии постов о железодефицитной анемии от меня, как от врача лабораторной диагностики, будет разбор показателей эффективности лечения анемии. Сегодня поговорим об этом. ⠀ Буду очень благодарна вам за поддержку в виде лайка ❤❤ ⠀ Только начну я с просьбы: пожалуйста, не рекомендуйте в комментариях друг другу препараты для лечения того или иного заболевания. Название, дозировку, кратность приема и способ введения препарата назначает врач на очном приеме, учитывая тяжесть состояния пациента и сопутствующие заболевания. То, что помогло одному из вас, не обязательно поможет другому, а может и навредит. ⠀ А сейчас к теме. Главное при лечении железодефицитной анемии: 🎯 найти и устранить причину дефицита железа 🎯 сделать запас железа в организме ⠀ Лабораторные критерии эффективности лечения препаратами железа. ЧТО ДОЛЖНО БЫТЬ при правильном лечении? 1⃣ ↑ ретикулоцитов (RE) на 7 день от начала лечения на 2-3%. Сейчас этот показатель оcпаривается, так как RE могут быть ↑ при кровотечении в организме. 2⃣ ↑ Дельта гемоглобина (D-Hb) - альтернатива пункту 1⃣. D-Hb – это разница между средним содержанием гемоглобина в ретикулоците и эритроците. ↑ D-Hb на 7 день после начала лечения. 3⃣ ↑Hb к концу 4-й недели лечения минимум на 10 г/л и гематокрита (Ht) на 3% по отношению к значениям до лечения. ❗Если перечисленного выше на приём препарата железа в указанные сроки нет, то консультация врача ОБЯЗАТЕЛЬНА. ⠀ Реакция организма на приём препаратов железа может ОТСУТСТВОВАТЬ при ⛔ неадекватно низкой дозе препарата железа ⛔ заболеваниях желудочно-кишечного тракта (нарушено всасывание железа) ⛔ низком уровне общего белка ⛔ анемии, НЕ СВЯЗАННОЙ с дефицитом железа. ⠀ Длительность лечения анемии составляет 1,5-6 месяцев (зависит от тяжести анемии). ❗Помните, что нормализация Hb не является основанием для прекращения приема или ↓ дозы препарата железа на 30-50%. Организму надо сделать запас железа. ⠀ Ферритин при приеме пероральных препаратов железа проверяем после курса лечения (через 7-10 дней). ⠀ Неужели остались вопросы по железодефицитной анемии 😉? My haemoglobin using this nifty non-invasive is not bad at 13.1 g/dl, however, at that point, my ferritin was 50, and I was experiencing some low memory and I couldn't sustain my energy levels with my best efforts. Most people see an OK haemoglobin and conclude that all is well with Ferritin (Iron), B12 and Folate. Not true! Before your haemoglobin takes a tumble into the red zone (depending on your lab ranges), it will have already taken a tumble from somewhere up there in the black, to somewhere down there *but still in the black*. So the FALL in haemoglobin is just as important to pay attention to, over time. This leads me onto my latest godsend, iron infusions! Top tips: 1. Please, in short, ensure your ferritin is checked, as part of an iron screen. The other markers are TIBC, Iron saturations and Haemoglobin. 2. Don't mess with oral iron which will improve serum iron, sometimes too dangerously. Why? If your absorption is poor, this iron dumps onto your organs and over the long term causes organ failure. 3. Animal iron is still much better absorbed than plant iron in most of our ancestry. BLOOD WORK WEDNESDAY 💉. I’m going to dedicate Wednesday each week to post about blood tests that are relevant to hair loss. Over the past several years, I have had many blood tests. Despite having several ‘abnormal’ test results, they have not been flagged by my doctors, and I have had to follow up, or research the interpretations of them myself. . I’m not blaming doctors for this, as they probably didn’t think that these results indicated a ‘health’ problem for me. However, I wish that I researched further, or pushed more for an explanation of my results. After completing the first 3 modules of my trichology program, and reading posts from leading dermatologists, I now have a better understanding of some of these blood test results. I’ll start with a shorter post since I’ve blabbered enough already and Instagram won’t let me write more. . Ferritin! This is one of the most common blood tests that is performed on women with hair loss. Ferritin is a measure of iron stores in the body. Low ferritin levels can possibly be related to hair loss, even in the absence of anemia. While I have read that iron levels of 50-70 ug/L are ideal, some dermatologists like their patients to aim for a level above 40-50 ug/L. A leading dermatologist in Canada, @donovanmedical has written several posts about testing ferritin in women with hair loss. In a recent post, he provides a great explanation about the possible relationship between ferritin levels and hair loss. He says “the short answer is that the lower a person’s ferritin is - the more likely it’s related to the hair loss they are experiencing. It’s a scale from “very likely related” when the ferritin is down below 15-20 to very likely unrelated when it’s above 40. There is no exact rule”. Over the years my ferritin has ranged from 5 - 45 and I have never been told to supplement. . For the longest time I was an ice eater, I’m talking midnight trips to the freezer to snack on a tray or two lol. I have really thick hair and ever since I can remember I would lose a handful when washing but my hair never felt like it was thinning. I was always tired and achey. Eventually after a blood test it was revealed I was severely anemic. Taking iron supplements has helped to bring my levels back to normal and reduced the amount of hair I loose. However some people find that the introduction of a new medication can actually cause more hair loss even if it is not listed as a side effect. Talk about catch 22! As endurance athletes we obsess over our food. Most of the time we are concerned about the right calorie intake and carbohydrate to protein ratios. However, most athletes neglect the importance of iron in our diet. Why is iron important? Red blood cells contain hemoglobin which is an iron-containing protein.  This protein transports oxygen to your muscles when you run.  If you have low iron levels, you will generate fewer red blood cells therefore transporting less oxygen to your muscles and hampering performance. To help maintain proper iron levels it's important to add iron rich foods into your diet. Yes, you can gain iron from veggies, legumes and fortified cereals. However, you need to consume considerably more of these sources of iron for it to be absorbed in your body.  The best way to improve iron absorption in your body is to eat lean meats, specifically red meat.  I personally have never been someone who craves protein but realize I need it to perform at my best. My go to red meat meal is lean ground beef with seasoning, brown rice, spinach, green peppers, avocado, tomato, sprinkle of cheese, dollop of plain Greek yogurt and sometimes I'll add black beans. Who takes an iron supplement? Any recommendations? I used to be able to take iron bisglycinate (gentle, non constipating) but even that messes my stomach up these days! Currently using @spatoneuk but not sure if my ferritin is increasing on it as I’ve not checked yet, it works out quite expensive as I take 2 a day so am open to alternatives, also I’ve seen @luckyironfish is meant to be quite good too, anyone tried it?
Друзья, и все-таки, что нужно определять для диагностики латентного дефицита железа: ФЕРРИТИН или РАСТВОРИМЫЕ РЕЦЕПТОРЫ ТРАНСФЕРРИНА? ⠀ По комментариям и вопросам поняла, что надо обсудить эту тему. Спасибо большое за Ваши лайки ❤, они помогают понять актуальность темы. ⠀ Напоминаю, что при латентном дефиците железа (ЛДЖ) 🔹гемоглобин в норме 🔹в норме или незначительно снижен уровень железа в крови (норма для мужчин – 13-30 мкмоль/л, для женщин - 12-25 мкмоль/л) 🔹МСН снижен (норма 24-27 пг); MCV снижен (норма 80-100 фл). ⠀ 🔴 Главный лабораторный признак ЛДЖ – низкий уровень ферритина. Низкий – это ниже 40 мкг/л (нг/мл). Если у Вас при низком ферритине есть жалобы на ломкость ногтей, выпадение волос, сухость кожи, заеды в углах рта, утомляемость, раздражительность, мелькание мушек перед глазами, сонливость, то ДЕФИЦИТ ЖЕЛЕЗА НАДО ЛЕЧИТЬ. Само по себе это состояние не пройдет. ⠀ Что надо помнить перед выполнением анализа на ферритин? ❗Уровень ферритина в крови имеет суточные колебания до 80%. Поэтому кровь на ферритин надо сдавать утром. ❗Ферритин уже некоторые лаборатории определяют в капиллярной крови. ❗Ферритин является белком острой фазы воспалительного ответа. Поэтому если у вас есть воспаление, инфекция, системное аутоиммунное заболевания, онкологическое заболевание, с высокой долей вероятности ферритин будет высокий. Он не покажет ЛДЖ, даже если дефицит железа есть. Для примера: при целиакии, ревматоидном артрите, болезни Крона и др. ферритин определять не надо ❌ ⠀ 🔴 Какой показатель может быть альтернативой ферритину? Растворимые рецепторы трансферрина (sTfR). STfR - это белки, которые располагаются на поверхности клеток и обеспечивают перенос ионов железа внутрь клетки. При латентном дефиците железа количество STfR в крови ПОВЫШАЕТСЯ. ❗STfR не реагируют на воспаление, инфекцию, онкозаболевание. ❗STfR - альтернатива ферритину у пациентов в возрасте и у пациентов с анемией хронических заболеваний. ⠀ В галерее нормы ферритина и STfR для детей и взрослых (сайт референсов www.aruplab.com). ⠀ Мой ферритин 43 мкг/л, расскажите, какой у вас ферритин? My friend Amalia had to stop giving blood because she became anemic. She shares her incredible health story with #ANEMIA and plexus and reminds us to donate too! “Two numbers: 14.2 and 4:50 14.2- that was my #hemoglobin today- a safe count to give blood. Giving blood has been important to me since I was in college. In college giving blood meant a better grade (extra credit) 😆 Now it means giving back. Every 56 days a person is allowed to give a pint of blood. I try my hardest to stay on that schedule. About 3 years ago I arrived to give my donation and my hemoglobin was too low. Not only was it too low to give blood but I was urged to go see my doctor as soon as possible. So I went to the Dr, we talked about dietary #iron and #ferritin . I told her how we probably eat too much red meat - at least 5 times a week and that spinach and broccoli are my favorite veggies. She ordered up some labs and my ferritin was way too low. I was told to go to the pharmacy and pick up an #iron supplement. I took the supplement as instructed and returned for lab work in 6 months. No improvement. I was told to double the dosage and come back in 6 months. Small improvement. I was told to keep going at that dosage and come back in 3 months. During that 3 months I began a supplements called Plexus to give me balanced all-day energy. Not only did it do that but it brought my iron up. How, you may ask?? ANEMIA and HASHIMOTOS = No Bueno It’s time for me to get back on track because when I’m my healthiest I “FEEL”my best. I had been feeling pretty awful the past month and with my history of anemia (low ferritin) and Thyroid disease my husband made me go get it checked sure enough he was right and I’m at an extremely low level again. Some of the symptoms of low iron levels are Brain Fog 🧠, Weakness, Fatigue, Restless Legs, Ringing in the ears 👂, Hair Loss 🤦🏼‍♀️, Brittle Nails, Heart Palpitations ♥️, Shortness of breath, increase anxiety, chronic fatigue are just to name a few. I’m feeling ashamed of myself right now. For the first time in a very long time Teresa couldn’t find a vein in either arms. She looked at my hand and thought she could see a vein but couldn’t promise anything. She tried my right hand and unfortunately it didn’t work. Soon afterwards I then felt poorly and went pale and felt sick. I don’t normally feel poorly after blood tests but I feel it’s because she tried my hand and that’s why I was ill. She was concerned that I looked pale and asked what I had for breakfast. She asked how urgent were the bloods. I’m therefore going back for attempt 2 which hasn’t happened for ages next Wednesday last one of morning surgery. I can’t believe my veins let me down for the first time in a long time. I really wanted to see the dr next week but that won’t be happening till probably the week after now. I’m just wondering if my ferritin level has dropped and that’s why it didn’t work today. Last Friday I got some blood work done and I got the results back today. My ferritin level was twice what it was last year! TWICE! I couldn’t believe that! Went to see my functional medicine doctor and found out I was low in vitamin D and iron. I wasn't surprised as most people are low in vitamin D. #Eisenwerte zwischen Norm und Realität 🧐 ⁉️Wie viel Eisen braucht unser Körper? Ab wann sprechen wir von einem Mangel und was zeigt uns der Test? . • 🇩🇪In Deutschland sind die Richtlinien folgend: 👱🏻‍♀️Ferritinnorm bei Frauen: 15 – 250 ng/ml 👱🏼‍♂️Ferritin-Norm bei Männer: 50 – 500 ng/ml . • 🇦🇺Amerikanische Forscher sehen das anders: Bereits bei weniger als 40 ng/ml spricht man hier von Eisemangel! ❗️Bei einem Wert unter 30 ng/ml handle es sich von einem milden Eisenmangel und bei einem Wert von unter 15 ng/ml von einer schweren Blutarmut! . • In der Realität sieht das meistens nochmal anders aus. Die Menschen sind individuell und wie das so ist und wie die Forschung das immer mehr und lauter zur Öffentlichkeit bringt, passen wir NICHT in eine standardisierte Norm. . • ✅Je nach Lebensphase brauchen wir mal mehr mal weniger Eisen ( z.B. Periode des Wachstums, Schwangerschaft, nach einer Krankheit...) brauchen wir mehr Eisen und dies deutlich mehr als nur um die 40 ng/ml. . • ❓Was ist DEIN optimaler Ferritin-Gehalt? . • ✅ Die persönliche Untergrenze kann individuell berechnet werden: Sie ist gleich dem Körpergewicht. D.h. wenn ich i.d.R. um die 50 kg wiege, heißt es, dass ich mind. 50 ng/ml haben muss. Optimal wäre für mich und für die meisten Frauen höher/gleich 100 ng/ml. . • ✅ In der Praxis wird empfohlen, sich an den Werten 70 – 230 ng/ml zu orientieren. Menschen, die um die oder häher als 100 ng/ml haben, fühlen sich vitaler, strahlen Gesundheit & Energie aus, sind kreativer und optimistischer. . • ❓Wie erfahre ich meinen Wert? . ❗️Den Wert kann man nicht berechnen oder fühlen, hier muss man unbedingt einen Bluttest machen. Leider wird das nicht von KK übernommen, was eine absolute Frechheit ist. Genauso wenig Vitamin- oder Mineralanalysen. Der EisenCHECK von @zuhausetest zeigt an, ob der Ferritingehalt im menschlichen Blut bei mehr oder weniger als 20 ng/mL Ferritin liegt. My favorite brand of iron supplements 🤩 I love this @newrootsherbal iron because it is heme iron! This makes it more bioavailable to the body, and as some one with habitually low iron my blood needs all the help it can get 😂 Nothing makes me angrier than the ranges of “healthy” ferrritin (aka iron storage) levels according to Western medicine. I often see levels in the teens and 20’s for many patients and their doctors don’t even mention it! And many of these patients do exhibit symptoms of low iron levels like fatigue, paleness, weakness, shortness of breath, and dizziness. In fact, I like to see ferritin levels closer to 90 or 100!
